Sending Cases


For Tissue Cases:

1. Fill out a UCSF Requisition Form and include a copy of the patient's insurance card. (Optional: remove yellow copy and keep for your record.)

2. Fill out the labels included on the requisition form and attach them to each specimen bottle.

3. Place tissues in formalin bottles and place them in a biohazard bag. Fold the requisition form and insert it into the pocket of the bag, then insert the bag in a shipping box or tube container. (Note: if using a tube container, wrap the requisition form around the tube container.)

4. Place either the box or tube container into a FedEx Clinical Pak, seal, and attach a completed pre-printed FedEx Priority Overnight Airbill.

Call FedEx for pick up or drop off at a FedEx location.


Notes For Immunofluorescence:

For direct immunofluorescence, place tissue specimen into the green direct immunofluorescence vial that we provide. Store at room temperature.

If H&E is also requested for histology correlation, include tissue placed in a formalin bottle.

**DIF bottles are filled with Michel's solution, different from the formalin bottles, and expire as indicated by the date on each bottle.

 
 

For Slide Consults:

1. Fill out a UCSF Requisition Form or draft a Cover Letter to our providers. Please include a copy of the patient's insurance card as well as the prior pathology report. Patient history and any available pictures are appreciated. Pictures may be included in the package or uploaded via our Document Upload page.

2. Place slides in a slide container wrapped in bubble wrap, if possible, or use a bubble wrap lined manila envelope.

3. Place the specimens as well as all documents into a FedEx Clinical Pak, seal, and attach a completed pre-printed FedEx Priority Overnight Airbill.
